That kind of 'between the big and next toe' shooting oneself in the foot is quite common at shotgun clubs where often a shooter will rest the loaded gun on his foot while waiting to shoot. There is a natural muzzle resting spot right where that shot went. Often it clips both toes. Hunters also tend to stand and talk with the muzzle resting between the toes - it keeps the muzzle off the dirt.
That's where the expression "shooting oneself in the foot" comes from.
The best one Ive heard of is where the dick caught the hammer/trigger of his pistol while drawing it, shot himself down the entire length of his leg shattering the ankle...30 inches odd of hole...
