Third scenario the spool is the wrong one for the reel - mixed up. I have done this myself with different models of the same make.

I think there are two scenarios 1) The reel is timed incorrectly so the spool is moving too far in or not far enough out - the wraps should cross and hold the line back. 2) after making a cast there...
