    Is it a really long flimsy unsupported FP? I'd suspect, particularly with a bubba spring behind it, that it'll bend/flex as it flies forward, may even be clipping the FP hole.
    Do you store your bolt cocked as in the photo? that will weaken springs over time, they'll take a set. Was it the first shot you'd fired after storing it cocked?
    You can blue up/colour in with vivid the firing pin and/or the hole, then fire a few times and see if/where the blue has worn off, may give clues.
